What is transition planning?
Transition planning is the development of a plan to assure that students with disabilities will be able to lead rewarding adult lives. Planning should begin by age 14.

It takes place when parents, students, educators and adult service providers sit down together to develop a Transition Plan as part of the Individual Education Program (IEP) process. The IEP should contain transition-related goals and objectives that will help a student to learn skills that he/she will need for future environments.
