 Transition Assessment Forms
Attention Special Education Staff: The following statement can be used on CAMPUS when you are making a Transition Evaluation Plan for a student in St. Paul Public Schools. You will not need to use all of the instruments/procedures listed. Please delete the instrument names that you will not be using when you make your individualized transition plan. To determine the needs and current level of performance in transition domains associated with community participation, home and daily living skills, recreation and leisure, employability knowledge and skills and understanding post-secondary education/training options, the following instruments/procedures will be used: Transition Planning Inventory (TPI) with responses from the student, parent/guardian and school staff; and other measures such as community-based learning experiences evaluations; Interest Determination, Exploration and Assessment System (IDEAS); Accuplacer (2-year college placement test), PLAN (a pre ACT 4-year college placement test); Saint Paul College Evaluation; Career Assessment Inventory (CAI); ASVAB Test and Program for Exploring Careers; Adaptive Behavior Assessment System (ABAS); English Language Learner Adaptive Behavior Assessment; Reading-Free Vocational Interest Inventory-2nd Edition; Deaf/Hard of Hearing Life Skills Evaluation; Home Transition Survey; and the Ready to Manage Your Own Health Care? checklist.
