Transition PLUS Services helps students achieve their goals and objectives by providing case management, specialized education, training, and support in five key areas of transition. Using a team process, students receive individualized counseling, advising, and training in the following areas: home living, community participation, jobs and job training, recreation and leisure, and postsecondary education.
