Transition
PLUS Services (T+) is a district-approved educational program for
special education students, 18-21 years old, who need assistance and
support while pursuing transition goals and objectives.
Transition
PLUS Services helps students achieve their goals and objectives by
providing case management, specialized education, training, and support
in five key areas of transition.
Using a team process, students
receive individualized counseling, advising, and training in the
following areas: home living, community participation, jobs and job
training, recreation and leisure, and postsecondary education.
