Eligibility Transition Plus is a community-based special education program in Saint Paul Public Schools.  Service is available to students who need support meeting their special education transition needs after 12th grade.     Students must meet the following eligibility criteria: • Unmet transition needs and/or goals in their IEP • Have a current IEP and ESR • Have not accepted a high school diploma • Have completed 12th grade school year • Are able to travel independently in the community • Can function in community settings with minimal to moderate support   Transition Plus staff members work in partnership with a variety of community resources, agencies, and businesses to meet each student’s individual needs.   Diplomas will be withheld for students who enroll in Transition Plus.  The diploma date will reflect the date special education services are terminated.
